Below outlines how I will collect and use your information.
Personal information that will be collected
All the information you provide from the Tiger Art Therapy clinical questionnaire will be stored securely in my practice management software, ensuring I comply with GDPR. If you would prefer to complete the questionnaire on paper, this will be securely stored in a locked filing cabinet.
I use practice management software to record client notes, manage my diary and appointments, remind clients about appointments, invoice for sessions and send receipts. This avoids handwritten notes and ensures I comply with GDPR.
Smartphone
I will temporarily store your contact details in my password protected smartphone for the duration of therapy. After this time, I will erase your details.
Your name, email address, and any other information contained in email communication to frances@tigerarttherapy.co.uk is stored securely in Microsoft data centers. Outlook email by Microsoft uses industry-leading encryption for all messages you will receive and send to me.
Fit for practice
In the unlikely event of my incapacity to practice, your name and contact details may be shared with another therapist so they can contact you. And only if you are in therapy.
Supervision
This is a requirement of my registered bodies BAAT and HCPC and maintains ethical practice. It involves supervisor and supervisee (me) engaging in reflective dialogues, tasked with the examination of clinical practice, including visual images, verbal and nonverbal interventions and process.
Emergencies
Your well-being is my priority. In the event of your well-being or that of another is at immediate risk (as in our agreement) your contact information may be shared with an emergency service. I will always attempt to agree a way forward with you and to obtain your agreement in the first instance.
Erasing personal data
Within 1 month of therapy ending. I will delete electronic emails and data related to smartphone use, as listed above. I do not keep handwritten information, as outlined above, however, if you provide me with paper documents, these will be stored in a locked filing cabinet for as long as I am required to do so by law and as a requirement of my therapeutic registrations. When not needed I will shred the copies.
Your rights
To be told what information I am holding and withdraw your consent for using your personal data or request your information be erased (this may be declined whilst needed for professional reasons)
